Laboratory prescription forms and instructions

• From Patients (via Dentist Prescription Forms)

Patient initials/name, age, gender, and unique case number

Relevant clinical information provided by the prescribing dentist (e.g., shade, impressions, device requirements)

 

We do not collect unnecessary personal data such as full medical records, or unrelated personal details.

 

2. How We Use the Data

The information provided is used strictly for:

• Manufacturing and supplying dental appliances and prosthetics in line with the dentist’s prescription

• Communicating with the prescribing dentist or clinic regarding case progress or queries

• Maintaining legal, regulatory, and quality assurance records

• Processing invoices and payments

• Improving our laboratory services through case tracking and audit

• We never sell, share, or use personal information for marketing without consent.

 

3. Data Storage & Security

All prescription forms, case records, and digital files are stored securely (electronically or in locked storage).

Electronic records are kept on password-protected systems with restricted staff access.

Data is retained only for the minimum period required by law and professional standards (typically 10 years). After this period, data is securely destroyed.

Information is shared only with relevant laboratory staff involved in manufacturing the appliance. Where necessary, trusted third-party suppliers (e.g., milling centres, specialist labs) may receive limited case data strictly for completing the prescription. We do not transfer personal data outside the UK.

Dentists, clinics, and patients have the right to:
